Deaths of two WA patients awaiting heart surgery send shock waves through halls of Fiona Stanley Hospital

Two West Australians have died while on the waiting list for heart surgery at Perth’s biggest hospital in the past six months — in one instance after a scheduled operation was cancelled for a “higher priority patient”. The pair of deaths are understood to have sent shock waves through Fiona Stanley Hospital’s cardiothoracic unit, which has been left especially devastated by the relative youth of the people involved, who are understood to be aged below 60. Both patients were awaiting elective cardiothoracic surgery, operations that are separated into three categories depending on how urgent the procedure is considered. The most serious “elective” surgeries — category one — are procedures that should be undertaken within 30 days because they have the potential to deteriorate quickly to the point where it may become an emergency. Publicly available data on the WA Health website reveals that as of February there were 76 patients waiting for a category 1 cardiothoracic operation at FSH — with the median waiting time 117 days, nearly four times the 30-day limit. A source with knowledge of the situation said a lack of staff — theatre nurses in particular — had created a considerable backlog of patients awaiting surgery to correct serious heart issues. They said that on multiple occasions in the past year, as many as 20 patients waiting for cardiac surgery were occupying beds at FSH at the same time. That kind of in-hospital monitoring for extended periods ahead of an elective heart procedure was described as unusual and evidence of serious concerns that the patients’ health was at risk of deteriorating. South Metropolitan Health Service chief executive Paul Forden confirmed the two deaths but would not disclose the age of the patients involved nor how long they had been waiting for their surgeries. He said one of the patients was being “investigated for other non-cardiothoracic related symptoms and was not medically ready for surgery”. The other death, which occurred earlier this year, sparked a review. “This patient required complex surgery — a rare procedure that only one or two surgeons in each State are able to perform,” Mr Forden said. “Unfortunately a higher priority patient urgently required surgery, and this patient passed before a new date could be aligned. The clinical advice is that it is very unusual for a patient with this heart condition to pass in this way. “For family and friends of patients needing surgery, a death of a loved one is tragic, and we express our sincerest condolences.” The source said requests for additional theatre time for cardiothoracic procedures dating as far back as a year had been knocked back until very recently. “The medical administration have gone into panic mode and is now granting more operating time but there is still no ICU staff, no theatre staff,” they said, adding many healthcare workers in those kinds of specialised roles were foreign-born and had left during the pandemic. Mr Forden said FSH’s cardiothoracic unit dealt with some of the most complex heart and lung cases in WA. “Often the surgeries undertaken by this service involve multiple surgeons and each session is on average four to five hours given the complexity of surgery required,” he said. “Since December 2022, we have actively increased capacity for cardiothoracic surgery and have put on 20 per cent more operating hours each week. “Patient care and treatment is our absolute priority and is always our first consideration when determining which patients receive surgery first. “Cardiothoracic patients, are, by nature of their illness, very sick and may deteriorate quickly for various reasons and this is not possible to predict. “All patients are asked to immediately contact the service if their situation changes or deteriorates. If this occurs, these patients are admitted to the cardiothoracic ward for monitoring and emergency surgery. “Cardiothoracic surgeons discuss all treatment options and associated risks with each patient, so they can make an informed decision with their family.” Liberal leader and shadow health minister Libby Mettam said it was “extraordinary” that patients in a State as wealthy as WA were “literally dying waiting for surgery”. “We know that many of the State’s specialist support staff such as theatre nurses, ICU nurses and anaesthetic technicians left WA during the COVID border lockdown and haven’t returned,” Ms Mettam said. “It took six months for the international recruitment blitz announced in 2021 to start and the Refresher Pathways program that helps nurses and midwives return to the workforce was suspended for more than 18 months due to COVID. “This lack of urgency is now having a dire impact on some of our most critically ill patients and their families.” Health Minister Amber-Jade Sanderson said bolstering healthcare numbers — including specialist surgeons — had been a key priority for the McGowan Government and the overall workforce was 21 per cent larger than in 2017. She said 46 medical occupations had been added to the skilled migration list for 2022-23, including cardiothoracic surgeons, anaesthetists, anaesthetic technicians and enrolled nurses.
